The chances of that happening would be minimum. After all the end user should have little more sense about it but it is a good point to note.

In that case would it help if the gnd plane isolate from the ac pins on the pcb.

How about a fuse with a low current rating, yet high breaking capacity on the USB connector?

Or maybe disable the pcb controls when a usb is connected.
