Help Identifying a component.

Almost definitely a transformer for a switch mode power supply. They're usually custom, so you're probably not going to find much by looking up the number (I didn't).

Yes, SMPS transformer, 2 windings.

It says 8 pins there, but the rest seems to match. Do you think it's broken?

I'd say even that matches, if you don't count floating pins.
