CCTV Camera Repair Fortec DWS-6060H - Identifying component

Hello,
I have a CCTV Camera Fortec DWS-6060h. It contains two cameras - one is for black and white picture and the other one only for colour picture. The colour one doesn't send a signal. It turns out that two elements were burned out. One is tantalum capacitor on signal line and the second is strange component by Microchip. It is marked only with digits: 934 1031. I measured it and it is shorted between legs: 1,2 and 4. Does anyone of you know what kind of element is it? I am attaching photos of PCB and this burned out element.

One is tantalum capacitor on signal line and the second is strange component by Microchip.
It is not Microchip but Mitsumi: MM1031, Video Amplifier

Thank you :) Logo on chip leads to Microchip. This Mitsumi chip has VCC=5V and supply voltage in circuit is 12V. Now I have to only find something similar on higher Supply Voltage.

EDIT: I checked again supply voltage and it is 5V Line not 12V, I made mistake at previous measurements.
