I want to hide and show a block diagram depending on the variant I'm using. Is this possible?

Should be possible if you create two different schematic symbols: https://www.altium.com/documentation/altium-designer/design-variants#

Embed a different image into each one. May be other ways.

Yea the only way I know of is creating a new SCH symbol like thm_w said. One thing to be aware of is that you will get a big red cross over the image and the image colour will remain, unlike the SCH symbol body which will turn black & white
