How does this heatsink mate with a to-220?

The bolt hole isn't drilled.

Some are designed with spring metal U clip that presses tab against heat sink.

Got it from digikey and there was no clip included.

As danadak says; there is a metal U clip required.
The hole of the TO-220 device sits over the 'pimple' sticking out of the heatsink and the spring clip secures the device to the heatsink.
Absolutely a spring clip is required.

Yes, as you can order a few versions of the heatsink, but there is only one clip style.

The picture is not too detailed, but could it be the hole is already prepunched? So you just pop it out with a rusty nail and an old shoe, if drilling is no alternative.

You wouldn't want to have to drill through it BTW -- you can do it a few times, but the anodize finish will punish drills; it's hard even on carbide tooling.
