Open Traces Root Cause

We have been having this problem of open traces since a while now. Up on cross sectioning we saw low copper evidence. We noticed that the separation is parallel to both sides of the hole and we also noticed that it is on the same location on two different boards. Please see attached picture and any feedback as to how this could have happened would help me to great deal.

What process are you using to etch your boards. Where are you getting your stock copper clad and how well is it washed and prepared before printing artwork and etching ?

We use Strip-ETch-STrip for etching and It is a standard FR 4 material

The picture looks like a failure in the plating of a via. So it would be nothing with etching, but more like something in the plating / cleaning process, maybe the drills.

We did notice today that thickness of copper is lower than Class 2 requirement. We take PTH measurements on 1 panel per lot and did not notice anything unusual lately. Guesstimate is that it is an isolated problem limited to 1 or 2 panels. Low copper looks like a symptom and not the problem. What could have gone wrong that we are seeing these problems?
