I now seem to have issue with output stage, schematics show a test point where I should get 450Khz square wave but when I probed it just noise.

We need to find out what you mean by "noise". It still sounds like a triggering issue to me, but in any case please measure the peak-to-peak amplitude of this noise and report back. Also, please examine the noise at different timebase settings to check if the noise resolves into a real signal at one of the settings.

I've rechecked all voltage rails to the amp and have 5v, 12v, +&- 39v and +&- 75v. ALL of this is on LV side of amp.

Excellent - this is very encouraging.

One thought is that if there was a smoothing problem in the power supply would that possibly give me the correct voltage but with lots of noise?

Yes, that is possible.

Finally, could you post a picture of the schematic (circuit diagram) showing the point where you have connected the probe ground lead and the surrounding circuitry? Thanks.
