SMPS and Transformer Design Help

I am looking for a consolidated design cook-book for:
1. Linear supply
2. Flyback
3. Forward
4. Half/full Bridge
5. LLC
Can anyone suggest a e-book/link that has everything conceptually explained with design examples?

Good question, I would like to know as well ..

1. LM317
2. works alike boost converter. important primary and secondary windings polarity
3. works alike isolated buck converter.
4. works alike isolated buck converter
5. hard

Is there any book/link that explains how the equations are derived?
