The big problem I have with all these is that they tend to assume things about your application. I have been caught out by effects that were not mentioned at all in the textbooks as I was not doing a standard application.

Depending on what you want, there are some reference books that I found very good:

Switchmode power supply handbook, by Kieth Billings
Switching power supply design, by Abraham Pressman
Switch-mode power supplies; SPICE simulations and practical designs, by Christoph Basso

Each covers different aspects of the supply, with the handbook being more about the small, often forgotten details like inrush limiting, auxilliary power, and transistor drive circuits. The SPICE book is obviously skewed more towards getting a supply simulating and the design aspects.

Of course, no book is likely to ever deal directly with the application you're trying to design for!

I know the author, so he gave me a signed copy.  It covers both linear and switch-mode supplies, and covers most topologies.

If you want a superb book on SMPS theory/repair then take a look at the website of Jestine Yong.  www.jestineyong.com  He's Malaysian but was trained in UK in electronics.  The book is an ebook and you can have it in your hands within seconds - fantastic.  Hope it helps you.
