The chat log test is pretty useless unless you have it in a video capture form that can be overlayed on the video easily. Anyway, it too late, video already uploaded.

The origin of phrases is interesting to me.  This UK site:

says that "that's all she wrote" is a phrase of US origin that probably came from a 1942 country music song which was then popularized by US GIs in WWII, probably when referencing "Dear John" relationship breakup letters received by GIs overseas.

Dave, I got bored and wrote a small php script which converts irc chat logs like the one posted above to a subtitle file which is accepted by youtube.

If you want, go to Captions on the video and upload the subtitle.srt file that's in the attached zip file to the video and the chat log should show up to those who enable captions.

Source code for the conversion script is also included so if you'll want to do this again you can just edit the paths to the irc chat log file and the subtitle file and run the script.

On the Asiana crash, pilot error in multiple respects:

However, the autothrottle was only "armed" or ready for activation, she said.  In the 777, turning the autothrottle on is a two-step process – first it is armed, then it is engaged, Boeing pilots said."

BTW, there was an unnecessarily big scandal in the US about racism when a (now former) summer intern at the NTSB gave a San Francisco TV station the names he had for the Asiana pilots and the TV station was stupid enough to believe them and show them on the air.  Here they are:

Sum Ting Wong
Wi Tu Lo
Ho Lee Fuk
Bang Ding Ow

What _idiots_ at that TV station would believe those were correct?  Asiana claimed they're going to sue, but most people here believe they should piss off on that and pay more attention to pilot training instead.

On the Heathrow 787 fire, it wasn't one of the LiPo battery packs:

The summer intern did not give KTVU (the television station in question) the names, s/he confirmed them. KTVU gets a tip from J. R. Awesome, good idea!
Have uploaded, will see what happens.
Need to sync at the start though I suspect.
