Most bureaucratic policies have evolved over time in light of experience. That's probably because they didn't have your input.

What I meant that they probably do not know how to use their database.

This is what my ex neighbor (I moved) says to the police.  It is not against the law to cuss out a policeman (boy was I surprised the first time).  He would cuss at them so much and loud they would always send 2 cars to control each other.

I once called animal control and the police read me the riot act because I did not report their address (I reported mine and said they were behind my house) and a female animal control officer showed up ( they were suppose to be two officers).  Somehow word got to the sheriff and they came running then when it was over they read me the riot act for placing her in harms way  .  This helped me decide to move.

I do not know about other civil workers.  Do this at your own risk.

Just answer the questions directly. "No" and "No" like others have already suggested. No need to volunteer any other information or explanation. If they want further information, they'll ask. This is just part of their standard dip sampling. Nothing to worry about.

Just use the format Dave has presented.

They aren't interested in fancy wording or pretty presentation - just clear answers to the questions.

The purpose is to have an official record of your answers that could be used as evidence - if necessary.

I would throw in some basic explanation, as they probably have to put a reason why somewhere.  Like some form they file or something.   

Though I've always been curious about if those HV mosquito rackets mess up certain radio frequencies when activated.  For the price you get them at I doubt they undergo any kind of testing. Regardless, they're fun as hell!

Just answer the questions.   Don't volunteer any additional information.   It will only
 (a)  confuse them and/or
 (b)  invite further questions and/or
 (C) annoy them for making them have to read more than they needed to.

IF they need anything more - they will ask.

+1 - anything more can only add confusion - these are admin people who are establishing of it falls under certain regulations, they wouldn't know an electronic load from a mosquito bat.
 They have asked a simple question to which "no" is a perfectly valid answer.

Without reading the regulations referred to, but assuming it's about EMC, then strictly speaking, "Yes" would  be a correct answer to the first question, which would doubtless trigger a request to see EMC conformance documentation, though it is reasonable to assume it conforms with applicabe standards, so "No" would still be reasonable.

What happens if he says "no" to the second question, and the DUS (Device Under Scrutiny) appears to have had no compliance testing of any kind whatsoever?

Edit: hmm, it's a Japanese company, chances are "no" is the right answer. I wouldn't have bet my life on it had it been some no name Chinese product... It's one of the reasons why I opted to buy my Maynuo DC electronic load through a third party rather than directly online.
