Berlin AquaDom explosion - what went wrong?

It's like some of the crafty/tricky (slightly bordering on dishonest) datasheet techniques, that manufacture's sometimes use, to make semi-exaggerated claims, about their devices.

There have been many cases of "impossible" things happening. The Titanic sank, Flight 231 lost all 3 hydraulic circuits, and so on. It's easy to overlook a failure mode that bypasses all of the redundancy you engineered into something.
