That thing looks like an actual ground symbol. Probably not the best EE-related joke from XKCD.
But overall this game is awesome.

Where's the cave hidden? I explored everywhere from left to right, but didn't find it.

I personally did not find it either, but if you go to to the place where he can drop under the black stuff, there are some hidden caves there. You will have to navigate blindly though.
I hope someone makes a full map.

There's a hidden set of tunnels in the volcano. drop below the "lava" on the right shore and start moving left, but not up and you should eventually hit some caves. The map can help a bit.

Ok, I get the under ground part. But apart from that I don't get the joke? :-[
It's a single ground point, buried deep in the earth's crust. As if all return current in the world flows to that single point. As others have mentioned, not hilarious but for an Easter egg it made me smile.
Also notice they appear to be wearing ESD wrist straps?
