I'm about 7km away from the exchange with adsl2+ and get a crappy 3.5-4Mbps. My brother, who lives in a different exchange area gets around 20Mbps with just adsl.

I don't remember what the actual speed was when I changed to adsl2 but remember thinking "Is this it?!?!"
Man that sucks so bad. I got 3G wireless but on most days it's only a little over 2G. I hate that.
Start right now.

ISPs often wait until they see a demand for services in an exchange before they'll consider it.

ISPs love to fob off their own failings on "big bad,Telstra"!

I supposedly have ADSL2 but My speed isn't anything to write home about.
I can only suggest you "suck it up" till the NBN is connected.
Your ISP can then whinge about "big,bad,NBN"! :D

Aren't other companies allowed to but their own ADSL infrastructure in alongside the Telstra equipment?

Why don't you get a service from one of them?
Other companies can put their gear in the exchange, but the copper in the ground to my house is own by Telstra. I report a fault they log it with telstra to fix. There are 50 pairs to this estate, all spare pairs a faulty.
Telstra has neglected the basic infrastructure for so long it's falling apart, at least in this area. Like the NBN or not it was the only chance of getting  a  true competitor into this country. Don''t even mention wireless - 15m antenna mast needed for basic TV

If it makes you feel a bit better consider yourself paying $60/month for 8Gb worth of wireless (very laggy and goes unusable at times when lot of people yap on their phones) internet. I'm not living in the outback, Brisbane under 10 years old suburb, full of RIMs to save money...
