Questions on the gate drive circuit for power MOSFET

The figure shows the gate driver circuits for one of the three-phase legs.

Here few questions:

[ a ] Should the output driver pin, which is connected to the source of the power MOSFET, be connected to the "negative" (common) of the isolated PSU? In other words, can the isolated PSU, optocoupler, driver, and the source of the power MOSFET share a common reference point?

[ b ] If the output gate drive voltage swings between 0V and +10V, does the circuit need back-to-back zener diodes near the gate? Or, only single zener diode is sufficient?

[ c ] Please share if you know any limitation (drawback) or design considerations of the gate drive circuit.
