This could be something as simple as your printer settings like: Fit to Page.
Try Actual size.

Always print to blank paper first and then place some components on it to check printer and PCB CAD scaling is correct.

Under Output>Print select Page Setup

Change "Scale Mode" to Scaled Print, then chose 1.00 as your scale.

Make sure the Printer Paper is selected as the size you need.

Under Color Set, choose "Mono" (to give you solid black for the transfer.)

On the same dialog, click "Advanced",  here you setup what layer you want to print, and you may want to choose Mirror if the image is being flipped onto the board.

Hit Ok to all that, and then Print>>Print Preview and if anything isn't right, go back and sort it out in those dialogs previous.

Yep, that seems to ring a bell but it's so long since I set up Altium to print properly and I wasn't sure if CS had the same options.
Think you nailed it Harvs.

Yeah the dialogs are exactly the same, only the menu to open it is in a different place.

If it is possible to export an image to windows clipboard you can try the printer driver of this program: pcbexpand.exe . http://www.vouwbad.nl/pcbgrowing/

This program also let you make the traces thicker so you have more chance of success with the final pcb.
