PCB/PCBA mnuf. recommendations ? (high end)

Looking for recommendations for PCB manufacturing and assembly houses in ASIA.

Minimum capabilities should include:
- 3 mil trace / 3 mil space
- controlled impedance (Megtron-6)
- 12 layers or more
- uVias, Blind & Buried Vias
- via back drilling (reliable!)
- X-Ray capabilities
- assembly of 0201 size components and very fine pitch BGAs (<0.4mm), top & bottom
- Must offer small prototype runs (5 pieces), and small (100pcs) production runs

PS: Would also be interested to hear from independent PCB layout guys who have their own Altium license.
