PTH padstack creation

I've been searching around for something that resembles a definitive answer for creating pads and through holes on footprints.  The best thing I've been able to come up with is that it varies depending on insertion and soldering method so the answers are usually pretty vague. 

For hand stuffed and soldered boards the best that I can figure is to go ten thou over the nominal lead diameter for the finished hole with a pad double the finish hole diameter and let the annular ring fall as it may.  Is this a reasonable method  for good results or is there something else that might be preferred?
