I found a file folder full of these FORTRAN coding forms (http://chrisgrossman.com/graph_paper/fortran_coding_form.pdf) in a collection of old graph paper. They are for use in entry on a keypunch machine.

I used those by the pound back in '70 and '71. Thanks for the PDF!
I built an FPGA incantation of an IBM1130 primarily to run FORTRAN jobs. There's just something cool about that machine.
