Reference of a good KVM?

Dual DVI and combining USB + PS/2 are both rare, never mind the intersection of those two sets. Could you compromise by either using VGA/DVI-A or converting PS/2 to USB with a passive or active converter? Probably if you can find anything that works, it will be discontinued since no mainstream computer ships with PS/2 and DVI anymore.
