LED blowing off

My LED on the board keeps blowing off. what might be the error?

We'll need much more info. First thing to check, is to make sure you're not breathing or blowing on it. That certainly could cause a part to move.

Could be a few things.  Not enough paste for the part to stick to, PNP not pressing the LED into the paste, blow setting on PNP during place is too high, place delay too short, place height too high and PNP is dropping the part.  Hard to say with limited info but those are the first things that come to mind.
