If you are asking about fabrication, there are plenty of cheap board fabricators (
http://pcbshopper.com/ is your friend) around that could do it for you. However, you will likely need the board in a Gerber format, not as a PDF.
Did you try to contact
http://www.qrpkits.com/mmr40.html ? They had this as a kit, now retired, but they perhaps could be willing to share the documentation and the design files with you. If not, it is not too difficult to redraw it from the schematics and the existing layout file above using e.g. KICAD so that you can send it out to have it fabricated.
