Never used such equipment, so I can only guess.
not sure why is insertion loss so high on pairs 4/5 and 7/8 when it is only a 4-wire setup; dtx is setup for 100base-t...
The 100base-t setting probably still tests those pairs. They are sometimes used (eg when people split the one cable into two and run two 100Mbit links over it)
High insertion loss == wire doesn't exist. Makes sense.
received a split pair error....; not sure if this definitely means that connector is miswired
Looks like it (EDIT: or it's a badly/non twisted cable). Both ends too, so the same person has done both. Have a look at the order of the coloured wires that have been punched down or crimped.
Without knowing more about the equipment: all of its results may be useless if the pairs are split.
Does it come with a manual?
