Is this a good enough Input stage for industrial use case?

Hi,
I want to apply an isolated 220V AC mains to a system, is this input protection enough? Note: I have to use the least amount of components in here, Since the design needs very Small PCB area!

Even if the 220 volt AC source was isolated, I would want galvanic isolation unless the 220 volt AC source was an isolated winding on a transformer which only went to the measurement input.  If I wanted to just measure presence of the 220 volts AC, then an optocoupler would be a good choice for isolation.

1. Is D1 a bridge rectifier? That doesn't provide isolation.

2. What are the power and voltage ratings of R1, R6?

3. What are the voltage ratings of R2, R3?

4. Do you have enough space for creepage and clearance?

5. What happens if D3 fails?

It's for measuring the AC, and the input is a winding on an isolated transformer,All resistors are 0805 and they have a 150V working voltage and D1 is 1000V bridge rectifier
