Datasheet needed for a box High Voltage power supply HVA1085

Hello All,

I have tried on Google with no success!

This is a  8cmx5cmx3cm box power supply sealed in epoxy.

from Advanced High Voltage Corp. Van Nuys, CA.

Model :  HVA 1085
Input : 20/40 VAC
Output : +1kV

With the picture below. Any help on the specifications and pinout is welcome.

Really ?  Nobody can help me for connecting properly this device ?

I could experiment by trial and errors, but with 1kV output, errors may be paid in an excessive manner.
