Does anyone know what Analog Devices IC this is?

Hi,
 I was working on an Analog Discovery board and found a bad IC.

I am not able to find what the component is. It may be some sort of regulator/boost IC.

Pins on a good part read
Pin 1 4.95V
Pin 2 4.95V
Pin 3 4.95V
Pin 4 0V
Pin 5 0V
Pin 6 3.31V
Pin 7 3.31V
Pin 8 1.40V
Pin 9 2.93V
Pin 10 15.71V

Any help identifying this part would be appreciated.

Well, that makes me feel stupid. That never seems to work for me so I don't even try anymore. Thanks for the simple advice and for linking to the part.
