Help identifying 3.5mm connector part number

Hello!

I need some help with identifying the part number of the 3.5mm audio connector on this printed circuit board.
I've looked through Digikey and Mouser and couldn't find a direct match for it. 

It's a PCB from the front panel of a Fractal Design Define PC case. Only after a year of use the audio output connector has stopped working properly.

Any help would be appreciated!
