Help Identifying this SSOP4

I have 2 boards about a year apart with these SSOP4s which I believe are opto-isolators.  One is marked "1 [] 724" and the other is "1 [] 648" like this:  (So I believe the 3 digit must be the date code)

Can anyone come up with a datasheet?   I suspect the board is applying too much voltage across the output.
