Part number although close, does not exactly match any of the examples on the website you found and the drawings look slightly different that the one in your photo, particularly the orientation of the gearbox.
My guess, this is probably a custom part manufactured for a specific OEM. If this is for a repair, have you tried contacting the manufacturer of the equipment to get a replacement?
What exactly is wrong with it? Most common failure of such motors is lack of lubrication, that can usually be easily fixed. If it's a stripped gear then that might be more problematic.
