Tascam DR-40 headphone jack part number

I got a Tascam DR-40 that was missing the headphone jack. Since the part is missing, I do not have a good idea what to look for.  I looked at hundreds pictures of jacks that are currently available and none of them look like an exact match.  Could someone tell me the manufacturer and part number for the jack?
