TDA8953TH / Large DC offset causing shutdown. Need help

I'm working on a portable PA (Yamaha) that is going into to shutdown from presumably the DC offset I am seeing on the outputs. It uses a TDA8953 as the power amp and my question, not having a ton of Class D chip amp exp.) is;

Would this much DC on the output indicate a failed chip?

1. About full rail on output 1
2. About half rail on output 2
3. About 3/4 rail on input 1, but on the "chip side" of the coupling cap.
4. About 1/8 rail on input 2, again, on the chip side of the couple cap.

You have a faulty IC and good luck to find a new and genuine one.
