Well, likely it can be taken apart and repaired but it will be very troublesome as it's most likely potted.

Looks like a hybrid IC, how old is the device?

The good news is that it looks like a casual switching power supply, judging by all the surrounding transformers and the optocouplers.  Maybe the whole thing can be replaced with a generic SMPS with the same output voltage.  Try to look at the silkscreen, or try to get a service manual in order to find the outputted voltages and their max currents.

What type of equipment is it?  Manufacturer and model number?

Maybe a single in-line relay, or resistor networks.

Well, I looked again and I noticed that the OP said it's a SMPS, but couldn't find any IC on the board, not even a BJT or MOSFET, so it's most likely a thick film IC module.

IMHO it's just a potted circuit on regular FR-4. Maybe a hybrid on ceramic substrate, but I don't think so. Those usually have a hard coating and no enclosure.

Imho really demands reverse engineer to have its vicinity schematic
is in primary or secondary part ?
