Help! Stuck for IC data

To save a life, would anyone in the know decode this IC for me?
I cant even identify the logo. The legend says LS4001S.

LS —> ON Semi Conductor is the manufacturer

If you search IP4001S you’ll see it’s a 5 channel motor driver IC
