What do Class III/IV Dielectrics, Ceramic Capacitors Look Like?

I read that these high-K, barium titanate or "barrier layer effect" capacitors are no longer standardized by the EIA and no longer made. But since they were once made, they could be lurking in old equipment. How do you recognize them?

I was unsuccessful in finding any pictures online. Anyone? Bueller?
