Reverse engineering analog IC

Hi,

I'm currently trying to reverse the NE556 from that picture and I don't understand some layout design.
Here is a simple npn transistor that I identified:

And this one looks pretty weird for me.

If this is npn bjt what a hell is that middle red zone? p+ doped or what?
